By submitting this form, you, as parent/guardian of the above athlete, give your permission for your child to participate in the evaluation for 1787 Lacrosse.  You understand lacrosse is a contact sport  that is very physical and do not hold the coaches, affiliates or school liable for any injuries that may occur during the evaluation.
